  2. We will meet you at the hospital/facility, your home, our office, video conference, or phone conference.

  3. Complete a Service & Care Agreement, and HIPPA Release form if needed. The Service & Care agreement outlines your individualized services, and pricing.

  4. Patient Advocate Rates: First visit up to 2 hours is $250, and each additional hour is $125. Before we meet with you, we will conduct a FREE discovery call. During our initial call we will give you an estimate of the time required to assist you.

  5. Care Management rates are based on each clients individual needs.
